Membership Plans

Membership Guidelines:

• Membership is valid for one year from the payment date and will automatically renew on that same date every year unless a request for cancellation is made and verified 30 days prior to the renewal date.
• The membership plan is per individual.
• Family members include a spouse or significant other (with the same address), and dependent children/adults up to the age of 21.
• Membership fees are non-refundable.
• Membership benefits cannot be carried over to a new benefit year.

Limitations / Exclusions:

• The membership plan is not insurance. The plan cannot be combined with dental insurance or any other special offers.
• The membership plan is not transferable to other persons.
• Care Credit cannot be used for membership payment.
• Only services provided at Phoenix Dental Studio are covered – treatment done by a referred specialist is not covered.
